We understand that entrusting a loved one with any care provider can be a daunting thought. Working alongside the social workers and commissioning bodies responsible for their care, we work together with families to see what opportunities might be available to best support your loved ones needs, and provide a step-by-step overview of how our Referral process works …
Initial Contact
To start the referral process, your local authority or social worker supporting your loved one will reach out to us directly. They will provide us with information about your family member or friend's needs, enabling us to determine if we can support them.
Exploring Options
After receiving the referral, we’ll be in touch to gather more information about your loved one and to share details about our available services. This step helps us understand if we can provide the necessary support and allows us to explore potential options.
Assessing Needs
We believe it's crucial to have a comprehensive understanding of your loved ones specific needs in order to find the right fit. To achieve this, we conduct a series of assessments which involve meeting and getting to know everyone better to find out how we can best support. Alongside this, we also listen to your wishes, and answer any questions you may have.
Finding the Perfect Fit
We take great care in matching your loved one with the perfect home and environment. Once we are confident that we have found the right fit, we create a support package with family input, that is tailored to the individual's specific needs and desires.
Supported Transition
Transitioning to a new home can be challenging, so to facilitate a smooth move we provide a planned transition period. During this time, you and your loved one have the opportunity to get acquainted with their new home, support team, and surroundings, which you can be involved in every step of the way.
Moving In
Finally, your loved one is assisted to settling into their new home! Our dedicated team will be there every step of the way, ensuring that they have everything they need to thrive and offering continuous support.
